An overview of nurse-assisted interventions. The Care Coordination of Liver Disease (CCoLD) initiative is a pilot project of a nurse-led model of care at Blacktown Hospital in Western Sydney, Australia. The service directly engages with patients and their carers to coordinate and deliver patient-centred, early, evidence-based, and structured interprofessional collaborative clinical care with emphasis on self-care actions
